(Fort Worth, TX) — State inspectors say an elevator accident that severely injured a nurse at John Peter Smith Hospital was caused by a worn-out brake.

A report from the Department of Licensing and Regulation says investigators discovered a lack of maintenance on the elevator.  Carren Stratford was permanently injured when the elevator crushed her in January.

The Fort Worth hospital has severed ties with Thyssen Krupp Elevator, the vendor that was under contract to service the elevators at the time of the accident.
